These are necessary forms and materials for candidates for graduate degrees. Because the forms become a part of your permanent record, it is important that they be typed or legibly handwritten.

Forms Required:

  • Application for Degree,  Diploma Page and Final Examination
    • Two originals of the form must be submitted to the Graduate Records Office 3 weeks prior to the date of the examination and by the current semester deadline for graduates. Your committee composition must meet the Graduate School requirements. Graduate Faculty status is provided on the Graduate School website. Provide each committee member with a copy of the exam request.
    • It is the responsibility of the student and major professor to contact all committee members, including the Dean's Representative appointed to the general exam for scheduling an appropriate final exam date.

*In addition to these web forms the Survey of Earned Doctorates, UMI Publishing Agreement form, and doctoral exit survey must be obtained from the Office of Graduate Records, 114 David Boyd Hall and submitted with the final document.
